Office Policies
Cancellation Policy
We require at least 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee. We understand emergencies happen — please call us as soon as possible if you need to change your appointment.
Payment Policy
Co-pays and deductibles are due at the time of service. We accept most major insurance plans, cash, checks, and credit cards. Please bring your insurance card and a valid photo ID to every visit.
New Patients
New patients should arrive 15–20 minutes early to complete paperwork. Please bring any relevant medical records, imaging results, and a list of current medications. A referral may be required depending on your insurance plan.
Telehealth
We offer telehealth appointments for follow-up visits and certain consultations. A telehealth consent form must be completed prior to your virtual visit. Contact our office to learn more about eligibility.
